Registration Process

1.   Complete and Submit

CAB to complete ADG Form and submit the relevant scope form​s together with the requirement documents. No registration fee required.

• (ADG) Conformity Assessment Body Registration Form
• AD1 Certification Body Registration Form
• AD2 Inspection Body Registration Form
• AD3 Calibration Laboratory Accreditation Form
• AD4 Testing Laboratory Accreditation Form
• AD5 Proficiency Testing Provider Registration Form
• AD6 Validation & Verification Body Registration Form
• AD7 Medical Testing Laboratory Registration Form
• AD8 Personnel Certification Body Registration Form

2.  Evaluation and Verification

An Evaluation Panel will evaluate completed form and verified the required documents. The process will take 1-6 weeks.

3.  Approval

Recommendation from the panel will be submitted to the Director of PSABD for decision. Approved CAB will receive letter of registration.
